When asked such questions, I think of Shirley Jackson (author of "The Lottery" and "The Haunting of Hill House," among others). When faced with a school application for one of her children, back in the 1950s, which asked for 'Color', she wrote in the space, "I prefer turquiose."

Only on the Census. I don't think I've seen that question ever asked on a job application. It's illegal here.
I answer Harris polls regularly and they always ask. And I am always honest. I have nothing to hide. :-)
Some forms here might ask Country of Origin ? But I don't think that is asked for employment.
