Excellent question. My guess is that far fewer people suffer long term health damage from anorexia than obesity. But that's because the anorexic are vastly out-numbered by the obese (I think).
Everything I am reading is pointing to bone density problems as being the most long term. For the rest of it, it depends on if the anorexic ever recovers, and how long they were anorexic for.
Neither is good, they are both eating disorders which can kill you if taken to extremes.

Anorexia will kill you much faster, and is harder to recover from.
There is good evidence that anorexia is strongly linked to childhood sexual abuse.
