It would depend on their speaking style, mannerisms, and context. Most likely I would take it to mean 'angry' unless otherwise indicated.
The first three answers.

pissed = drunk
pissed off = angry
In Great Britain it means drunk I think and in the U.S. it means angry.
After that last grenade I was so pissed on I couldn't make it to the grogbog.
