I basically add any DVD that Netflix recommends to me to my queue and then randomize my queue once I hit the 500 movie max. The result is a steady stream of movies that I like, without having to make decisions about what to watch.
I read reviews, talk to my friends, I'm more interested in movies directed by my favorite directors or movies that have my favorite actors, and I test the trailer to see if the movie looks any good.
A fail of any one of these may lead to almost certain death for the movie in my book. Although, I must admit, the Kung Fu Panda trailers were quite possibly the worst I've ever seen [almost, but not quite], but the movie was wonderful. So it doesn't work 100% of the time.
Depends. I tend to trust the director. i am a Shyamalan fan, for instance, and find his movies enjoyable even when flawed. Otherwise, I go by reviews. If it is fantasy and/or SF and gets bad reviews, it is probably a good movie--mainstream critics still have no idea what the genre is about.

I rely on friends. I read reviews. If the friends say "yay" and the reviews say "nay" I trust friends more than reviews.
